Eligibility
- The nominee must be a tenured faculty member.
- Neither emeritus faculty nor university officials at the rank of associate dean or above are eligible for the award.
Nomination Requirements
- Nominations are accepted from department chairs. If a center, institute, or other unit wishes to advance a nomination, it should coordinate the nomination with an academic department.
- Each department may submit one nomination per year.

Each nomination should include a nomination letter (approx. 5 pages) and curriculum vitae.
Please address the following:
- Educational contributions: courses taught, course development, textbooks and other educational materials that have been disseminated, educational awards, training of students in research (undergraduate or graduate – please relate the number trained to norms in the field and discuss trainee outcomes), outreach activities
- Research contributions: scholarly/artistic impact, publication record (provide a short description of the impact of 3 publications from the nominee, as well as overall number of publications if the publication list is extensive), funding record (please relate to norms in the field), awards and external recognition, invited talks
- Service contributions: UW service (departmental, college/school, and university level), external service (funding agencies, editorial positions, professional societies), research leadership positions, diversity and inclusion efforts
Eligibility: Full professors with a record of outstanding scholarship and service for 10 years or more at UW-Madison.
